Use the release of records form to release transcripts, testing records, immunization, health, and other pertinent records of said student to Dupont Park Adventist School’s Admissions Office.

The recommendation forms is only one piece of the student’s profile to be used in our assessment process and will not become part of the applicant’s permanent record.
